Definition Affidavit
An affidavit is a written sworn statement of fact voluntarily made by an affiant or deponent under an oath or affirmation administered by a person authorized to do so by law. Such statement is witnessed as to the authenticity of the affiant’s signature by a notary public. The name is Medieval Latin for he/she has declared upon oath.
In the Philippines only lawyers may be authorized to act as a notary public.

Affidavit of loss is a legal document declaring the actual loss of government ID, company ID, drivers license, credit cards, insurance policy, share certificates, sim card or any other item through theft or destruction by fire/flood. It will contain full details of the loss, the owner’s name, address, and description of the items. The affidavit must be notarized by a notary public.
